2. If you want to purchase by check or money order, you will need to print out this page and complete it. Then, you can snail mail it to us at the address at the bottom of this page. (see below), along with your check or money order (in U.S. dollars). Please be sure to also include any applicable Shipping & Handling Charges and Sales Tax. 3.
